Book Benefactor List & Form

The Book Benefactor program provides an opportunity for you to donate a book of your choice to the school library. Your book will include a bookplate with inscription of your choice. You can give the book in your name or your child's name.
Bus & Walker Passes
Bus Passes authorize the school to let your child take a different bus than their usual bus. Please note the bus stop location must be an existing bus stop on the bus route.  Daily bus pass forms must reach the school office by 2 p.m. the  school day before the change takes effect. Permanent bus changes must be approved through the Transportation Department using the following link: Online Permanent Bus Pass Request. Please also send in a bus pass noting it is permanent  to the Douglas School Office once you have received approval from Transportation.

Walker Pass/Pick-up Forms authorize the school to dismiss your child in a nonstandard way. For example, use a walker pass when your child wants to walk home rather than take the bus or when you need to pick up your child early for a doctor's appointment.

CORI Forms

CORI Forms are available at the Douglas School Office. Any person who wants to volunteer in the Douglas School for the current school year must have a CORI (criminal offender record investigation) check. CORI checks are mandated by the state for the protection of your children. We recommend that BOTH parents/guardians submit CORI forms.

Douglas at Dawn/Dusk

Douglas offers Before & After School Daycare and Enrichment Classes called Douglas at Dawn/Dusk. Read the Douglas at Dawn/Dusk Policies and use the Registration Forms on the Douglas at Dawn/Dusk page to sign up.

Emergency Dismissal

The Emergency Dismissal Form lets us know your dismissal preferences if emergency conditions require school dismissal prior to regular dismissal time. We must know your child's destination. All students must return a Signed Emergency Dismissal Form at the beginning of the school year. All Walker Passes and Bus Passes will be cancelled in the event of an Emergency Dismissal.   Questions regarding Douglas' procedure may be found at Emergency Dismissal Procedure

Lunch Forms

School Lunch is available Monday, Tuesday, Wednesday, and Friday in the Douglas Cafetorium. Students can purchase school lunch which includes milk or milk a la carte. Lunch costs $2.50. Milk costs 50¢. Lunch Menus are online. Applications for free or reduced lunch are available online and in the school office. Students who were eligible for the previous school year can receive free or reduced lunch for the first two weeks of school. Parents MUST complete a new application for the current school year. To prepay your child's lunches, use the ParentOnline website or send a check to the cafeteria with this Elementary Lunch Form. Cash is also accepted at the register. Regardless of payment method, all students must use a Lunch ID (available in PowerSchool) to purchase lunches.
